Atlantic Health System Registered Nurse, AHSIC-AtHome Medical Day Shifts 8am-4pm Morris Plains, NJ in Morris Plains, New Jersey

Responsible for working with other healthcare professionals to help treat patients with various injuries, illnesses or disabilities in the home. Monitors patients, provides education on DME equipment (VPAP, oxygen and feeding pumps), keeps records, consults with healthcare providers, educates patients and more.

You will be part of the continuation of care to support the transition from hospital to home providing education to clients with respiratory, ventilation and g-tube medical issues. Nurses provide care to clients on a rolling case load basis. Nurses get to see many different patients consistently keeping the position interesting and rewarding.

Entry level pay rate is $41.50/hr paid biweekly. The travel expectation will include the use of your vehicle to the below territories, but not limited to them. Nurses are paid the federal guidelines of $0.65/mile to travel to client’s homes. On-call is required in this position, Nurses are paid a flat rate unless they need to provide care (get on the phone with a client or go see a client) at which time they are paid their hourly rate. On-call schedules are a minimum of 3 days per month.

Territories:

Sussex County, Morris County, Warren, Passaic, Union, Hudson and Bergen Counties.

    Principal Accountabilities:

    1.Travel to homes to provide education on DME, Durable Medical Equipment.

    1. Maintains accurate, complete health care records and reports.

    3.Provide education on oxygen therapy, instructions on VPAP therapies, as well as feed pumps.

    1. Monitors, reports, and records symptoms or changes in patient conditions.

    2. Assesses, implements, plans, or evaluates patient nursing care plans by working with healthcare team members.

    3. Documents observations and significant findings and communicates to other care team members in patient record.

    4. Provides support and education for patients with a pulmonary diagnosis as they transition from hospital to home on ventilation support.

    5. Excellent with problem solving skills and able to multitask.

    6. Utilize teach back to demonstrate, teach and supervise provided to patients and caregivers.

    7. Performs other related duties as assigned.

    Qualifications

    Required:

    1. Bachelor's degree in Nursing.

    2. 2+ years of experience in Nursing facility or home care.

    3. Current state licensure as a Registered Nurse and BLS certification required.

    Preferred:

    1. Trach/Vent experience- adults and pediatrics

    2. NG/G-tube experience -adults and pediatrics

    3. Knowledge of different feeding pumps.

    4. Knowledge and experience with VPAP therapies.
